Have you ever noticed small, firm bumps developing on the back of your neck after a fresh haircut or shave? If so, you might be dealing with AKN (acne keloidalis nuchae).
While it can be frustrating and sometimes uncomfortable, understanding the causes and treatments can make managing AKN much easier. Let’s dive into what AKN is, what causes it, and the best ways to treat it.

What Is AKN?
AKN, or acne keloidalis nuchae, is a chronic skin condition that primarily affects the back of the neck and scalp. It begins as small, itchy and sometimes tender bumps, often mistaken for simple razor bumps or acne. However, over time, these bumps can grow larger, become inflamed and harden into scar-like formations called keloids. This is where AKN gets its name.
Though more common in men with coarse or curly hair, AKN can affect anyone regardless of hair type. It’s also more prevalent among individuals who frequently shave their heads or have short hairstyles that expose the back of the neck. The condition can be persistent, but with the right treatment, its progression can be controlled and even reversed.
What Are The Symptoms Of AKN?
The symptoms of AKN can vary in severity and may develop gradually. In general, however, here’s what to look out for.
Early Signs - Small, firm, itchy bumps appear on the back of the neck, often where the hairline meets the scalp. These bumps can feel tender to the touch.
Progression - The bumps may enlarge, becoming inflamed or filled with pus, indicating an infection. This can lead to discomfort and even pain.
Scarring - Over time, untreated bumps can harden into raised, keloid-like scars that are more noticeable and difficult to treat.
Hair Loss - AKN can also lead to permanent hair loss in the affected areas, as the inflammation damages hair follicles.
If you notice these symptoms, especially after a haircut or shaving, it’s crucial to address them early to prevent scarring and manage irritation.
What Are The Main Causes Of AKN?
Several factors can cause AKN. But the most common include skin injury from irritation, immune reactions in the cells of hair follicles and extra androgen hormones.
1. Skin Injury From Irritation - Skin irritation is the most common trigger for AKN. Frequent shaving, especially with razors, can cause tiny cuts and abrasions on the skin, leading to inflammation around the hair follicles. Similarly, close haircuts that expose the back of the neck, combined with friction from clothing, hats, or helmets can aggravate the condition. Over time, these injuries create a cycle of inflammation, bump formation and eventual scarring.
2. Immune Reactions in the Cells of Hair Follicles - For many, AKN is caused by an overactive immune response. When hair follicles are damaged, the body’s immune system reacts to repair the area, but this can lead to excessive inflammation. This response may cause bumps to form and, in some cases, result in scarring.
3. Extra Androgen Hormones - Hormonal imbalances, particularly elevated levels of androgen hormones, can also play a role in AKN. These hormones increase oil production in the skin and stimulate hair growth, which can clog hair follicles and contribute to irritation and inflammation. Individuals with naturally higher levels of androgens may be more prone to developing AKN.
